Registration packets to apply for Sulphur Springs Independent School District’s Head Start, pre-kindergarten and kindergarten programs for the 2009-2010 school year will be available from 8:30 a.m. to 2:30 p.m. Monday-Friday, Feb. 16-20, at Early Childhood Learning Center, 390 Hillcrest Drive.
The Head Start Program is for children who reside in Sulphur Springs ISD, will be either 3 or 4 years old on or before Sept. 1, and who meet certain other requirements.
Among the services Head Start provides are certified teachers and assistants, two nutritional meals and a snack per day, full day educational services, outdoor playground facilities, transportation services, social services, parent involvement activities, speech and hearing evaluations, health/dental screenings. The program also accepts children with mild, moderate or severe disabilities.
When picking up registration packets, parents and guardians will be required to present a number of items, mostly official documentation of the child and family’s needs and residency. Standard registration items include the child’s birth certificate, Social Security card, up-to-date immunization record, custodial documents or court orders and doctor’s notes concerning any allergies or health needs. Current Medicaid, private insurance or CHIP cards must be presented as well. If the child has an individualized education plan (IEP), that information must be presented when securing an application packet. If in foster care, that information, too, is needed.
Parents and guardians should also have proof of all household income, which can include tax returns, W-2 forms, paycheck stubs, TANF, Social Security Income, child support, alimony, military documents such as LES/FSSA. Where applicable, food stamp cards and case numbers are also needed.
One proof of residency which shows the child resides in the Sulphur Springs school district must be presented when picking up applications. Residency can be established using a water bill, electric bill, gas bill or other utility statement for the household.
Parents will also be asked to schedule an appointment to return the completed application to the school.
Parents may also pick up registration applications to register their children for kindergarten or to see if their children qualify for the pre-kindergarten program.
Pre-kindergartners must be 4 on or before Sept. 1, and the family must meet certain income requirements to be eligible. Kindergartners must be 5 years of age on or before Sept. 1.
Pre-k and kindergarten applications are filled out at home, then returned completed with appropriate accompanying documents to the school as soon as possible.
Parents of these potential students are also asked to bring certain documentation with them when they pick up the pre-k and kindergarten applications. Applicants are asked to provide proof or residency; the child’s birth certificate, Social Security card, individualized education plan (IEP), doctor’s notes concerning any allergies or special health needs the child may have; custodial documents and court orders affecting the child, when applicable; foster car information, if applicable; and the child’s current Medicaid, private insurance or CHIP card.
Pre-kindergarten applicants will also be asked to provide food stamp card and case number, LES military information, TANF documentation, Social Security Income information, and proof of household income. Tax returns are the preferred proof of income, but the most recent check stub for anyone who brings income into the household is acceptable.
